De chartervlucht en de bemanning
Een vice president van een bedrijf voor equity trading had via NetJets een vlucht geboekt van Columbus naar Naples, Florida. Omdat de gebruikelijke vliegtuigen bezet waren door de Super Bowl, ondercontracteerde NetJets de vlucht aan Hop-a-Jet. De bemanning bestond uit een captain met 10.380 uur, een first officer met 24.581 uur en 116 uur op de Challenger 600 series, en een cabin attendant van 27 jaar die geen officiële certificering had maar wel vrijwillig een veiligheidstraining gevolgd.
Eerdere hung starts en corrosie
Vóór de vlucht beschrijft de spreker een incident op 15 januari 2024, toen beide General Electric CF34 motoren van N823KD niet naar idle power wilden accelereren. De piloten noemden dit een hung start en stopten de motoren. Technici controleerden brandstof, verving filters en volgde aanbevelingen van General Electric, maar vonden geen afwijking. De spreker legt uit dat het vliegtuig eerder in Barbados en Fort Lauderdale in een zoute, vochtige omgeving had gestaan en dat corrosie in de variable guide vanes was ontstaan. Door beschadigde spindels konden de variable guide vanes niet volledig sluiten, waardoor bij start en idle te veel airflow in de compressor kwam en de operating line dichter bij de stability limit line kwam.
Dubbele motoruitval en crash op Interstate 75
Tijdens de aanloop naar runway 23 in Naples zette het autothrottle de motoren terug en daarna weer voorwaarts. De spreker zegt dat beide compressors toen boven de stability limit line gingen, waardoor beide motoren tegelijk roll back tot sub-idle. De first officer riep uit dat er geen motoren waren en dat ze moesten landen, en meldde een emergency aan Naples Tower. De captain zei dat hij hier zou landen en wees naar Interstate 75, terwijl de first officer waarschuwde voor verkeer. De cabin attendant verliet de cockpit, bereidde de cabine voor en bracht de passagiers naar het baggage door. De jet raakte twee auto's, sloeg tegen een sound barrier wall en ontbrandde.
