De video begint met een fragment uit het middagnieuws van KTVU in Oakland op 12 juli 2013. De anchor las vier namen voor als pilootnamen van Asiana 214, terwijl de echte namen al eerder bekendgemaakt waren. Na de uitzending volgde een correctie en later excuses van het station.

In de video wordt uitgelegd dat de anchor de namen van de prompter las en dat een producer de NTSB had gebeld. Een zomerintern zei dat de namen volgens hem klopten. Het station erkende later dat het de namen niet hardop fonetisch had nagelezen en dat het te snel ging.
Aanloop en crash van Asiana 214
Op 6 juli 2013 vloog een Boeing 777-200ER van Incheon naar San Francisco met 307 mensen aan boord. De crew kreeg toestemming voor een visual approach tot runway 28L, terwijl de glide slope buiten dienst was. De piloten waren Lee Kang-Kook en Li Zhongmin, met een relief first officer in de jump seat. Tijdens de aanloop liep het vliegtuig te hoog en te snel, ging de autothrottle naar hold, vielen de PAPI-lichten op vier wit en later vier rood, en werd er pas laat go-around gegeven. Het toestel raakte de seawall kort voor de runway. Drie passagiers, drie tienermeisjes, kwamen om het leven; 49 raakten ernstig gewond. De brand was na ongeveer 50 minuten onder controle.
NTSB-intern en nasleep
De NTSB noemde in haar probable cause onder andere mismanagement van de descent, onbedoelde deactivatie van automatic airspeed control, ontoereikende monitoring van airspeed en een vertraagde go-around. De NTSB noemde ook de beschrijving van autothrottle en autopilot, de training bij Asiana en crew fatigue als bijdragende factoren. Bij de nieuwsuitzending had een zomerintern online gezocht en gezegd dat hij geloofde dat de namen klopten. De NTSB bracht later een excuus uit omdat het geen crewnamen bevestigt. KTVU ontsloeg drie producers, Asiana trok haar klacht terug, en de intern was niet meer bij de NTSB.

4:49 At 11.21 in the morning, the crew was cleared for a visual approach to runway 28L. In the left seat flying the airplane was Lee Kang-Kook, 45. He had more than 9,600 hours as a pilot, but only 33 hours in the 777.
7:29 500 feet, engine still at idle. The NTSB says the approach wasn't stabilized. No go around. 400 feet, the instructor, on glide path, sir, two white, two red, but the airspeed keeps falling below their 137 knot target.
9:47 49 were seriously injured. The fire was under control about 50 minutes after impact. In one note about the three teenagers, not a one of them had their seatbelt fastened when they came in to land. And the NTSB does say that could have made a huge difference.
12:04 KTVU's station manager had a different version, that the intern said the station could confirm the names through the NTSB, just don't use his name. According to the same statement, the intern didn't disagree and said he may have used the word confirm.
14:34 Asiana announced it would sue, not the NTSB, the station. The airline said the report not only disparaged Asians in general through the use of racially charged epithets, but it also severely damaged the reputation of Asiana Airlines.
